To provide clinical/case management services to Jefferson Center consumers as deemed appropriate by the guidelines of accepted clinical practice within the home environment of Karen’s House.  To provide information, education, liaison, and transition services to various networks within Jefferson Center and with federal, state, and community based agencies and resources. To provide protective oversight, medication monitoring and care within the home environment.

 

Essential Duties:

  • Provide clinical/case management services including service planning, monitoring, follow-up, and crisis management for consumers.
  • Make appropriate referrals to, and act as liaison with community agencies, service providers, and natural support systems
  • Collaboration with consumers, family members, and significant others in identifying strengths/needs in developing personal treatment goals
  • Work with consumer and family/support system for appropriate service delivery
  • Provide assessment of daily living skills (ADL’s), coordination, monitoring, planning, referral and follow-up with comprehensive community services and supports
  • Provide individual, group, family case management services and therapy as indicated and identified in the service plan
  • Provide assistance in developing community-based resources for social, vocational, and leisure skills
  • Maintain communication and links with other Jefferson Center programs e.g. Access, Inpatient, Outpatient, Older Adult, and Adolescent programs
  • Maintain appropriate paperwork in timely manner, including admission paperwork, 27-65 paperwork, treatment plans and required Dept. of Health paperwork
  • Ongoing monitoring of assigned caseload, assessment of symptoms to prevent relapse, and follow-up with respite services as needed
  • Coordination for emergency services when clinically indicated/medically necessary; risk/lethality assessment (including inpatient admission)
  • Resource acquisition and assistance with application for benefits and entitlements in collaboration with Benefits/Navigation Team
  • Assist consumers with basic needs such as clothing, medical and dental care, legal, vocational, and psychiatric services
  • Participate in team meetings, clinical staffings, and clinical consultation as scheduled
  • Utilize clinical practice guidelines, Center and State Office of Behavioral Health standards for appropriate service delivery
  • Facilitate treatment and provide structured living environment to residents
  • Supervise daily routine, events, activities
  • Assist with Activities of Daily Living as indicated for each consumer, while providing required care in areas of laundry, cooking and cleaning
  • Monitor medications and track MARs (Medication Administration Records) to ensure accurate delivery/documentation
  • Cross-training with other Center residential programs for coverage purposes
  • Maintain a safe, home-like environment, as well as protective oversight of residents and facility at all times
  • Schedule/participate in safety/fire drills and document in log book
  • Communicate all concerns in an appropriate/timely manner to supervisor
  • Develop and maintain relationships with neighbors in the residential community

  • Attend mandatory in-services, compliance with individualized training plan as required
  • Participate in supervision by coming prepared with an agenda.  Report high risk/problem cases, and utilize a problem solving approach/utilize feedback
  • Use language and behavior to promote dignity/respect with consumers, Center staff and external customers
  • Continually strive for self-improvement and is motivated to learn new skills to benefit consumer and Center
  • Attend supervision at times and intervals agreed upon with supervisor.
  • Satisfactorily complete 90% of peer review charts needing corrections within 14 calendar days (only areas for which staff is responsible)
  • Ensure that 95% of all authorizations are accurate and timely (insurance)
  • Submit 90% of all Progress-to-Date forms within 3 working days (team specific)
  • Complete 95% of all paperwork by the due date
  • Ensure that fee collections meet the team goal as determined by Network Director
  • Complete 95% of case closing within 90 days after last contact, 210 days for “Meds Only”
  • Meet required number of consumer service hours as determined by Network Director and/or Clinical Manager
  • Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment
  • Maintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public
  • Effectively respond to client/consumer needs and problems, initiate and maintain positive interactions, timely response to phone calls, pages, email, etc
  • Demonstrates cultural competency/awareness/skills in working with diverse caseload and utilizes Center resources/trainings as needed
  • Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor.
  • Volunteer to work on Center committees or community projects

Education, Knowledge, Skills & Experience Required:

  • High school diploma or GED required. Bachelor’s Degree preferred, with one to two years related experience and/or training.

  • Bi-lingual preferred, but not required.

  • Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determine by our insurance carrier. An MVR will be pulled along with a background check.
Salary:

Grade 49 – Below Bachelor’s

  • $35,500 to $45,200*
  • *Salary is determined based on years of total relevant experience
  • *Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

Grade 50- Bachelor’s Degree

  • $38,100 to $48,600*
  • *Salary is determined based on years of total relevant experience
  • *Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

**BONUS OPPORTUNITY!! $1,000 bonus paid out on one month anniversary. ** Based on full time (40 hours/week). Prorate if less than 40 hours/week. Must work a minimum of 20 hours/week to be eligible for bonus.
